Your records, your choice

Data retention & deletion requests. We keep adult records for 7 years after your last visit, and children’s records until age 25 (or 7 years, whichever is later), then securely destroy them. You can tell us how you’d like your records handled.

Your clinical records are kept in our practice software, Dental4Windows. Older paper record cards hold X-ray prints, item codes and notes; when their time comes, they’re shredded in accordance with the law.

3. Before you send

  • Identity check: we will verify your identity before acting, usually by phone or at the practice.
  • Timing: we respond within 30 days.
  • Limits: we can’t destroy clinical records before the minimum period or while a complaint, claim or legal matter is open – we’ll explain if so.
How we handle this request (privacy collection notice)
  • Who: Advanced Dental Healthcare Pty Ltd (Myaree Dental & Professional Centre), 84 North Lake Road, Myaree WA 6154.
  • Why: to confirm who you are and carry out your request about your records. If you don’t give your details, we can’t act on the request.
  • Where it goes: when online sending is switched on, your request goes over a secure (HTTPS) connection to Zoho Forms in Zoho’s Australian data centre. Your name, date of birth and contact details are encrypted in the form. Your name, date of birth, contact details and the type of request are then copied into our Zoho CRM (also in Australia) so our Privacy Officer can verify your identity and reply within 30 days; any notes you add stay in the form. Our notification email says only that a request has arrived; it contains no personal details. Nothing is saved on this device.
  • Who sees it: authorised staff only. We keep a record of the request and what we did, as required by law.
