How we handle your feedback (privacy collection notice)
- Who: Advanced Dental Healthcare Pty Ltd (Myaree Dental & Professional Centre), 84 North Lake Road, Myaree WA 6154.
- Why: to improve our service, for staff training, and as evidence for our practice accreditation. If you include your details and ask us to, we’ll use them to respond to you.
- Anonymous by default: unless you choose to include your name and contact details, we don’t collect them. Please don’t include detailed medical information. We only ask which types of treatment you had, and that’s optional.
- Not published: your feedback is for our team only. We don’t publish it or use it as a testimonial.
- Where it goes: when online sending is switched on, answers go over a secure (HTTPS) connection to Zoho Forms, stored in Zoho’s Australian data centre, and are seen only by authorised staff. If you choose Named and ask us to contact you, only your name, contact details and overall rating are copied into our Zoho CRM so we can follow up. We keep feedback for 2 years, then delete it. Your draft is saved only on this device until you send it or press “Clear my answers”. There are no analytics or tracking on this form.
